b0f7377944 Rewritten the synchronization mechanism between audio and video. 
Also:
- Updated AX_CXX_COMPILE_STDCXX m4 macros.
- Eliminated the emu_thread lock that I don't use.
- Fixed fast forward gfx_draw_screen().
